We also offer discounts for prompt payment of self-pay accounts.
Our physicians are providers on most of the major insurance plans offered in this community. Please check with your insurance carrier or employer to verify our participation on your plan. Most plans include co-payments, deductibles and co-insurance which is the patient’s responsibility to pay in a timely manner. Please call the office for information about specific insurance plans.
Some plans require a referral authorization be obtained from your primary care physician before you can be seen in our office. Please contact your insurance plan or primary care physician to make arrangements to bring this referral with you. If you do not bring it, we may have to reschedule your visit.
Please bring your insurance card with you to each visit. You should also let us know when your home address, employer, insurance plan or telephone number changes so we can update our records.
